MDS Registered Nurse (RN) - PRN

About the Role

The MDS Nurse is responsible for organizing, completing, and evaluating resident assessments and care plans in compliance with state and federal regulations. They coordinate the MDS, CAAS, and care planning processes, ensuring timely care plan development and accurate documentation. The role involves collaborating with residents, families, and interdisciplinary teams, monitoring resident conditions, and participating in quality improvement initiatives. The nurse also mentors colleagues, responds to crises, and maintains a professional, compassionate environment.

Responsibilities

• Coordinate assessment and care planning processes for new and existing residents
• Complete assessments, establish ARDs, and ensure documentation aligns with care plans
• Collaborate with physicians and team members to provide comprehensive care
• Participate in quality initiatives, surveys, and investigations of resident concerns
• Mentor staff, maintain professional standards, and support continuous improvement

Requirements

• Licensed Practical Nurse (LPN) or Registered Nurse (RN) with an active license in the practicing state
• Minimum of 2 years of MDS experience; skilled nursing experience preferred
• Strong communication skills, proactive in anticipating needs, and capable of fostering resident and team engagement
• Ability to adhere to policies, participate in audits, and support quality assurance activities
