About the role
Fletcher Rehabilitation and Healthcare Center is a Long Term Care facility seeking a MDS Coordinator to join their team. The position offers benefits including PTO, paid orientation, and new perks like health insurance, dental insurance, vision insurance, and a 401(k).
Responsibilities
- Keep abreast of current federal and state regulations, as well as professional standards.
- Absorb and develop comprehensive care plans for Residents in coordination with the MDS.
- Coordinate, manage, and monitor the written plan of care for each Resident of the facility.
- Assist nursing management with the coordination, management, and review of nurse’s notes.
- Monitor Resident status changes to ensure appropriate and timely nursing or clinical team involvement.
- Audit MDS and support documentation to ensure accuracy and compliance with regulatory and auditor requirements.
- Support the discharge process by assisting nursing management.
- Complete administrative requirements such as filling out necessary forms and reports.
- Maintain infection control and standard precaution practices when providing care.
- Follow established safety precautions when performing tasks and using equipment and supplies.
- Maintain strict confidentiality regarding sensitive health information of Residents.
- Report hazardous conditions, damaged equipment, and supply issues to appropriate persons.
- Maintain the comfort, privacy, and dignity of Residents and interact with them in a warm, respectful, and caring manner.
- Respond promptly and courteously to call lights in Resident care areas.
- Communicate and interact effectively and tactfully with Residents, visitors, families, peers, and supervisors.
- Attend and participate in departmental meetings and in-services as directed.
Requirements
Required Education and Experience:
- A current State License as a Nurse.
- CPR Certification.
Qualifications
Preferred Education and Experience:
- One year experience as a Nurse in a long-term care setting.
Skills
Knowledge and training in all aspects of MDS process.
