MDS Nurse

We are looking for a skilled, compassionate Full-time MDS Nurse who desires a career that matters in a community that values their expertise. This role is a critical partner on the health care team responsible for all aspects of the MDS process, including care planning and meeting with families regarding the care of their loved one. Responsibilities

• Coordinate the development, completion, and transmission of assessments in accordance with regulatory requirements.
• Review and manage reimbursement processes.
• Case manage care plans, including rehabilitative programs.
• Demonstrate knowledge of clinical standards of practice, regulations, and reimbursement governing long-term care and Medicare participation.
• Lead and deliver high-quality services in a person-centered environment.
• Recognize the autonomy and dignity of all residents.
• Communicate a sense of caring, concern, and dignity for residents.
• Place decision-making in the hands of residents whenever possible and appropriate.
• Make prompt and accurate judgments regarding resident care and emergencies.

Requirements

• Current OH RN license.
• BSN preferred.
• Prefer 3 years of MDS experience in a long-term care/nursing home environment.
• Experience in post-acute environments is a plus.
