MDS Coordinator (RN) - Full-Time | Day Shift | On-Call Weekends

About the role

This position is responsible for completing all MDS, admission, significant change, change of therapy, end of therapy, discharge, and re-entry tracking forms, as well as quarterly and annual assessments following the RAI manual. The role involves entering MDS data into a patient information system, transmitting it to CMS, and maintaining records for 15 months.

Responsibilities

• Ensure timely and accurate completion of MDS/CAA records and assessments.
• Coordinate with physicians and team members to maintain assessment accuracy.
• Contribute to care planning and participate in interdisciplinary problem-solving.
• Develop and implement resident-specific goals aligned with team objectives.
• Perform other assigned duties as needed.

Qualifications

• Graduate of an accredited nursing program with an active RN license.
• Preferred two years' experience with long-term care plans, MDS regulations, and CMS guidelines.
• Excellent verbal communication and organizational skills.
• Physical ability to stand, walk, lift, and perform documentation tasks.
• Capable of working independently and collaboratively within a team.

Pay

Compensation: $63,943 - $106,407 annually.
