About the Role
Teams with multiple departments to conduct and coordinate the development, completion, and updates of the resident assessment and care plan in accordance with state requirements and CLC policies. This position monitors and guides the completion of PPS, OBRA assessments, RAPs, and Triggers, and performs nursing functions as required. Participates in surveys and inspections by authorized government agencies.
Responsibilities
- Oversees and evaluates the gathering of information for assessments and care plans; schedules and facilitates care plan conferences.
- Monitors and guides the completion of PPS, OBRA assessments, Triggers, and RAPs.
- Maintains and updates written policies and procedures governing the resident assessment (MDS) and care plan.
- Transmits completed MDS to appropriate parties weekly; maintains 15 months of completed MDS records for surveyors.
- Assesses prospective residents in conjunction with other team members; works on an interdisciplinary team.
- Performs nursing functions when needed.
- Completes administrative duties such as medical forms, reports, evaluations, and studies.
- Develops and maintains an ongoing quality assurance program for resident assessments and care plans.
- Participates in on-site surveys by authorized government agencies.
- Other duties as assigned.
Requirements
- Registered Nurse with a current, unencumbered license in the state of operation.
- At least two (2) years of managerial experience in a hospital or nursing care setting preferred.
- Able to demonstrate knowledge and skills necessary to provide care appropriate to the age-related needs of residents.
- Schedule flexibility, including ability to rotate managerial on-call duties.
- Ability to problem-solve independently and knowledge of medical practices, regulations, and nursing care communities.
- Basic proficiency with office software applications including Word, Outlook, and Excel.
- Must be able to read, write, and speak English.
- Excellent customer service skills.
Working Conditions
- Light physical effort: handling objects up to 20 pounds occasionally and/or up to 10 pounds frequently.
- Sits, stands, bends, and moves intermittently during working hours.
- Subject to frequent interruptions.
- Communicates and interacts frequently with residents, family members, and visitors.
