MDS Coordinator (RN/LPN) – On-Site

About the role

This position involves coordinating resident assessments and care plans in collaboration with multiple departments, ensuring compliance with state regulations and organizational policies.

Responsibilities

• Oversee data collection and facilitate care plan conferences.
• Monitor assessments including PPS, OBRA, RAPs, and Triggers.
• Maintain accurate documentation and evaluate assessments.
• Update policies and transmit MDS reports.
• Participate in surveys and inspections by government agencies.
• Perform nursing functions and administrative tasks related to medical forms and evaluations.
• Support an ongoing quality assurance program to ensure assessment accuracy and compliance.

Qualifications

• Current, unencumbered RN license in the state.
• Minimum of two years’ managerial experience in healthcare settings preferred.
• Knowledge of age-appropriate care, medical practices, laws, and regulations.
• Strong problem-solving, communication, and customer service skills.
• Proficiency with office software such as Word, Outlook, and Excel.
• Flexibility to work rotating managerial on-call shifts.

Physical and Working Conditions

• Light physical effort; handling objects up to 20 pounds.
• Frequent standing, sitting, bending, and interactions with residents and families.
• Subject to interruptions and active communication with residents and team members.
