MDS Coordinator Duties
- Assure timely and accurate assessments of interdisciplinary care plans.
- Aid in identifying resident needs; communicate specific care needs & expectations to families.
- Utilize information from assessments to assist caretakers in formulating care plans, collaborating with social services, dieticians, rehab specialists, and medical staff.
- Implement and monitor care plans to ensure effectiveness and compliance.
MDS Coordinator Requirements
- Current license as a Registered Nurse (RN) in the state of NY.
- Understanding of CMI and ability to maximize CMI.
- Knowledge of Medicaid and Medicare.
