Responsibilities
- Coincides the assessment, planning, implementation and evaluation of resident care by adhering to the MDS process.
- Coordinates and facilitates care conferences with the interdisciplinary team.
- Cross-trains between PPS and OBRA.
- Conducts regular resident interviews as required by the MDS.
- Available to respond to clinical emergencies, regulatory agency visits, surveys, and issues outside of normal work hours as needed.
Qualifications
- Active RN license in the State of Connecticut, required.
- Knowledge of Quality Assurance/Improvement processes, required.
- Bachelor's or Associate degree in a health care related field, preferred.
- Experience in a supervisory position in long-term care, preferred.
- At least 3 months experience completing MDS and completion of MDS training program or equivalent experience and knowledge of PPS.
