MDS Coordinator RN

About the role

We are currently searching for an MDS Coordinator to join our friendly, caring and supportive team at Harmony Utica Ridge. Responsibilities

• Give your team the knowledge they need to care for each resident’s unique needs.
• Ensure our residents receive the high standard of care they have grown to expect by developing, monitoring, auditing, and modifying each resident’s care plan for their individual needs and goals.
• Perform resident assessments and assist in the discharge process.
• Ensure our residents are comfortable and safe by utilizing your knowledge, skills, and attention to detail.

Requirements

• Minimum of an unencumbered State of Iowa R.N. License.
• Graduate of an accredited nursing program.
• C.P.R. Certification.
• Prior experience as an MDS Coordinator is preferable but not required.
• One (1) year of experience as an R.N. in a Skilled Nursing Facility setting.
• One (1) year of experience as a C.N.A.
