MDS Coordinator (RN)

Responsibilities

• Cook up accurate MDS assessments in accordance with federal and state guidelines.
• Partner with nursing, therapy, social services, and other departments to develop individualized resident care plans.
• Ensure documentation supports quality resident care, regulatory compliance, and reimbursement accuracy.
• Monitor assessment schedules and update MDS documentation as residents' conditions change.
• Participate in care plan meetings and contribute clinical expertise to improve resident outcomes.
• Educate and support clinical staff on MDS processes, documentation standards, and regulatory updates.
• Work closely with leadership to maintain compliance and promote continuous quality improvement.
• Help ensure Complete Care at Brick continues to provide the highest level of resident-centered care.

Qualifications

• Average of 2 years of experience as a Registered Nurse-RN-MDS Coordinator or MDS assessment experience in a skilled nursing setting.
• Associate's or Bachelor's Degree in Nursing.
• Current, unrestricted New Jersey Registered Nurse (RN) license.
• Previous long-term care experience preferred.
• Strong clinical assessment, communication, and organizational skills.
• Knowledge of federal and state MDS regulations.
