Responsibilities
- Cook up accurate MDS assessments in accordance with federal and state guidelines.
- Partner with nursing, therapy, social services, and other departments to develop individualized resident care plans.
- Ensure documentation supports quality resident care, regulatory compliance, and reimbursement accuracy.
- Monitor assessment schedules and update MDS documentation as residents' conditions change.
- Participate in care plan meetings and contribute clinical expertise to improve resident outcomes.
- Educate and support clinical staff on MDS processes, documentation standards, and regulatory updates.
- Work closely with leadership to maintain compliance and promote continuous quality improvement.
- Help ensure Complete Care at Monmouth continues to provide the highest level of resident-centered care.
Qualifications
- Associate's or Bachelor's Degree in Nursing
- Current, unrestricted New Jersey Registered Nurse (RN) license
- Previous long-term care experience preferred
- Experience as a Registered Nurse-RN-MDS Coordinator or MDS assessment experience in a skilled nursing setting preferred
- Strong clinical assessment, communication, and organizational skills
- Knowledge of federal and state MDS regulations
