About the role

As the MDS Coordinator at Avalon Care Center Yakima, you will drive quality outcomes, ensure assessment accuracy, and support exceptional resident care in a 101‑bed facility located in sunny Central Washington. The position combines clinical expertise with leadership, offering meaningful impact on residents, families, and the facility’s success.

Responsibilities

• Oversee the MDS process and resident assessments.
• Ensure accurate, timely MDS completion and regulatory compliance.
• Coordinate care planning with interdisciplinary teams.
• Support Medicare and Managed Care requirements.
• Drive quality outcomes and reimbursement accuracy.
• Serve as a key clinical and operational partner within the facility.

Requirements

• Current Washington RN license (or NLC‑eligible).
• Previous MDS Coordinator experience.
• Strong knowledge of the RAI process and case management.
• Strong attention to detail and organizational abilities.
• Ability to meet all employment screening requirements.
