We are seeking a dedicated professional to complete and submit MDS assessments and tracking forms in compliance with CMS timelines and RAI guidelines. Key Responsibilities Include Coordinating assessment and care-planning activities with residents, physicians, and the interdisciplinary teamMaintaining accurate records to support resident-centered careEnsuring adherence to regulatory guidelines and documentation standardsSupporting the delivery of comprehensive, individualized care throughout each resident’s stay Qualifications And Skills Strong attention to detail and organizational skillsKnowledge of CMS requirements, RAI guidelines, and MDS processesExcellent communication and collaboration abilitiesAbility to work effectively within a team to promote quality resident care
