MDS Coordinator Responsibilities
- Cook up the maintenance and completion of Minimum Data Set (MDS) assessments, care plans, and documentation for our residents
- Collaborate with nursing staff, healthcare providers, and residents to identify and address changes in resident conditions and needs
- Develop and implement individualized care plans that meet the unique needs of our residents
- Ensure accurate and timely completion of MDS assessments and care plans
- Maintain accurate and up-to-date records and reports
- Participate in quality improvement initiatives to enhance the overall quality of care and services
MDS Coordinator Requirements
- Current Registered Nurse (RN) licensure in the state of Missouri
- Experience in a long-term care or senior living setting preferred
- Strong clinical knowledge and skills in geriatric care
- Excellent communication, interpersonal, and organizational skills
- Able to work effectively in a fast-paced environment and prioritize tasks and responsibilities
