About the role
Conceives and creates care plans based on a comprehensive resident assessment through resident and family interviews, physical assessment, observation, and input from the care team. Leads the MDS coordination process to ensure accurate and timely completion of all nursing sections of the MDS assessments and supervises nursing staff as needed.
Responsibilities
- Schedules PPS/MDS assessments for residents admitted to the nursing care center within established time frames.
- Compiles data for resident insurance updates for residents admitted with PPO and managed care payer sources.
- Communicates effectively with the Director of Therapy and Insurance Case Managers to maximize RUG level for reimbursement.
- Leads the MDS completion process by ensuring accurate and timely completion of all sections by responsible disciplines.
- Works effectively within the interdisciplinary team to ensure MDS process and coordination of care plans/assessments.
- Participates in the Nurse Management on-call rotation.
Requirements
- Must be a Registered Nurse currently licensed in the State of Maryland.
- Two years of long-term care experience.
- RAC-CT preferred but not required.
- Experience with PointClickCare preferred but not required.
- Must be able to follow and give accurate written and oral instructions and possess visual acuity to effectively prepare and read reports.
- Supervisory and leadership skills with the ability to communicate effectively with staff, residents, and visitors.
- Ability to coordinate and organize own work and the work of others to meet multiple job responsibilities and deadlines.
- Proficient computer skills; Microsoft Office experience preferred.
Schedule
Full-time: Monday–Friday 8:30AM–5PM, with rotating on-call duties.
Pay
$76,000 – $93,000, depending upon years of experience.
