Your Role as the MDS Coordinator
Americare Senior Living is seeking a detail-oriented and clinically skilled MDS Coordinator to support accurate assessments, regulatory compliance, and quality resident outcomes within our skilled nursing community.
Summary
The MDS Coordinator is responsible for coordinating and completing the MDS process as required by federal and state regulations. This role ensures accurate, timely, and compliant clinical documentation, supports care planning and Medicare processes, and collaborates with nursing leadership, the Administrator, and the interdisciplinary team.
Responsibilities
- Coordinate and complete MDS assessments in accordance with federal and state guidelines
- Ensure admission assessments, nursing notes, care plans, and required clinical documentation are accurate and current
- Complete assigned MDS sections, assessments, and RAP summaries
- Review, update, and maintain resident care plans based on assessment findings
- Participate in Medicare meetings, care plan meetings, and interdisciplinary team discussions
- Assist in maintaining compliance with state, federal, and Americare standards
- Support survey readiness and participate in inspections as needed
- Communicate changes in resident condition to the Administrator and nursing leadership
- Assess nursing supply and equipment needs
- Provide on-call support to staff as needed
Qualifications
- Graduate of an accredited school of nursing
- Current Registered Nurse (RN) license required
- Experience with MDS 3.0 and skilled nursing
- Strong attention to detail, organization, and analytical skills
- Ability to collaborate effectively with interdisciplinary teams
