MDS Coordinator (Registered Nurse/RN)

The RN MDS Coordinator is responsible for coordinating and assisting with the accurate and timely completion of interdisciplinary MDS Assessments, CAAs, and Care Plans in compliance with CMS RAI Manual Regulations and applicable laws.

Responsibilities

• Coordinate and assist with the completion and submission of assessments.
• Report patient condition changes.
• Educate staff on regulations.
• Review care plans and validation reports.
• Monitor assessment timeliness.
• Participate in interdisciplinary meetings.
• Perform staff nurse functions as needed.

Qualifications

• Associate’s or bachelor’s degree in nursing.
• Active RN license.
• Minimum two years of nursing experience, preferably in geriatrics.
