MDS Coordinator (Registered Nurse/RN)

Role: RN MDS Coordinator Responsibilities Coordinate and assist with accurate and timely completion and submission of interdisciplinary MDS Assessments, CAAs, and Care Plans in compliance with CMS RAI Manual Regulations.Review assessments, validate reports, and ensure assessments are entered into the CMS QIES system before billing.Report patient condition changes to the Director of Nursing and review the Interdisciplinary Care Plan.Provide education to care staff on CMS regulations and documentation updates.Participate in interdisciplinary meetings and review CMS Reports to prevent delays and errors.Perform staff nurse functions as needed and exhibit excellent customer service.Assist with patient evacuation and ensure safety protocols. Qualifications & Skills Associate’s or bachelor’s degree in nursing from an accredited institution.Active RN license in good standing.Minimum of two years’ nursing experience, preferably in geriatrics.CPR certification (obtained upon hire or during orientation).Advanced knowledge of post-acute care practices, laws, and regulations.Ability to make independent decisions, maintain confidentiality, and adhere to policies.Strong communication, interpersonal, and organizational skills.Ability to perform physical activities such as lifting and moving patients.Proficiency in English, with good reasoning and judgment abilities.
