MDS Coordinator Nurse

About the role

We are seeking an MDS Coordinator Nurse to lead and manage the RAI/MDS process and support resident-centered care as part of a collaborative IDT team.

Responsibilities

• Capture timely MDS assessments and ARDs
• Complete CAAs and care plan items
• Ensure documentation supports accurate MDS coding
• Participate in Medicare utilization management

Requirements

• LPN or RN licensure
• EMR proficiency

Preferred

• Long-term care experience
• RAI/MDS background
• PCC experience
