Essential Functions
- Keep abreast of current federal and state regulations, as well as professional standards.
- Absorb and assist with the development of comprehensive care plans for Residents in coordination with the MDS that accurately addresses the needs of the Resident.
- Cook up, manage, and monitor the written plan of care for each Resident of the facility that identifies the needs of the Resident and goals to be accomplished for each need.
- Assist nursing management with the coordination, management, and review of nurse’s notes to determine if the care plan is being followed.
- Monitor Resident status changes to ensure appropriate and timely nursing or clinical team involvement.
- Absorb MDS and support documentation to make sure they are accurate representations of the Resident and meet regulatory and auditor requirements.
- Perform regular audits of documentation to make sure they are accurate.
- Assist nursing management with the discharge process.
- Handle administrative requirements, such as completing necessary forms and reports.
- Absorb established infection control and standard precaution practices when providing care.
- Follow established safety precautions when performing tasks and using equipment and supplies.
- Maintain strict confidentiality regarding sensitive health information of Residents.
- Report all hazardous conditions, damaged equipment and supply issues to appropriate persons.
- Maintain the comfort, privacy and dignity of Residents and interact with them in a manner that displays warmth, respect and promotes a caring environment.
- Answer and respond to call lights promptly and courteously when working in Resident care areas.
- Communicate and interact effectively and tactfully with Residents, visitors, families, peers and supervisors.
- Attend and participate in departmental meetings and in-services as directed.
Requirements
- MDS experience
- Current State License as a Nurse
- C.P.R. Certified
- Knowledge and training in all aspects of MDS process
Qualifications
- One year experience as a Nurse in a long-term care setting
