Responsibilities
- Oversight and facilitation of resident assessments in compliance with Federal and State regulations
- Establishment and adherence to assessment schedules to ensure accurate and timely completion
- Transmission of assessments to the State of Connecticut, Department of Health
- Coordination of resident care plans according to regulatory requirements
- Initiation of Medicare coverage for newly qualified residents and issuance of denial letters when necessary
- Stay updated on Medicare coverage changes and assist in determining necessary reimbursement documents
- Coordination and facilitation of care conferences with the interdisciplinary team
- Care coordination and completion of Minimum Data Sets (MDS)
- Education and training of nursing staff and other departments on the roles of assessments, planning, implementation, and MDS process
- Ensuring all current MDS/PPS updates are implemented and staff is adequately trained
- Conducting regular resident interviews as required by the MDS requirements
Requirements
- Licensed Massachusetts RN or LPN
- Minimum 1 year of experience as a MDS Coordinator
- Strong communication, organizational, and assessment skills
