MDS Coordinator LPN/RN

About the Role

As an MDS Coordinator, you will coordinate the Minimum Data Set (MDS) assessment process, ensuring accurate and timely completion of assessments, and maintain accurate, up-to-date patient and resident records. You will collaborate with our medical team, families, and other healthcare professionals to provide comprehensive care.

Responsibilities

• Coordinate the MDS assessment process, including scheduling and facilitating assessments with interdisciplinary teams
• Ensure accurate and timely completion of MDS assessments, RAPs, and other required documentation
• Maintain accurate and up-to-date patient and resident records, including medical and social histories
• Collaborate with medical teams, families, and other healthcare professionals to ensure comprehensive care
• Develop and implement effective communication strategies to ensure seamless coordination of patient and resident care
• Identify and address any discrepancies or inconsistencies in patient and resident records
• Participate in quality improvement initiatives to improve patient and resident outcomes

Requirements

• High School Diploma or equivalent required
• MDS Coordinator experience preferred
• RN/LPN Nursing licensure
• Strong organizational and analytical skills, with attention to detail and ability to work in a fast-paced environment
• Excellent communication and interpersonal skills, with ability to work effectively with diverse stakeholders
• Ability to maintain confidentiality and handle sensitive information
