Your Role
- Complete and maintain accurate MDS assessments and documentation
- Develop, review, and revise individualized resident care plans
- Ensure accurate documentation for reimbursement from all payor sources
- Coordinate with interdisciplinary team members to support quality resident care
- Monitor resident progress and update assessments as needed
- Support compliance with long-term care regulations and facility policies
- Help maximize resident outcomes and appropriate resource utilization
- Maintain accurate and timely documentation related to assessments and care planning
- Support resident dignity, safety, and well-being
- Perform additional duties as assigned by management in a resident-centered care environment
Who We’re Looking For
- Current Illinois RN or LPN license required
- Long-term care experience required
- 1–2 years of MDS experience preferred
- Strong understanding of long-term care nursing practices and regulations
- Excellent communication, organizational, and documentation skills
- Dependable and professional team player with reliable attendance
- Passionate about providing compassionate care and improving resident outcomes
