MDS Coordinator (LPN)
About the role
The MDS Coordinator (LPN) is responsible for ensuring the accurate completion and submission of all sections and disciplines of the MDS, regulatory compliance, and comprehensive resident assessments. This role also involves leading care plan meetings, coordinating MDS/IDT/Care Plan meetings, and serving as a resource for staff.
Responsibilities
- Ensure accurate completion and submission of all sections and disciplines of the MDS.
- Stay up to date on latest regulations, conduct audits, and implement corrective measures to ensure maximum reimbursement through CMS.
- Implement and update all required assessments quarterly, annually, and as needed, ensuring accurate resident assessments.
- Initiate significant changes and corrections if discrepancies are noted in ADL coding or documentation/assessment requirements.
- Meet with staff to educate on RAI process, care planning, and resident assessments.
- Document findings from comprehensive assessments and evaluations, ensuring regulatory compliance.
- Complete signing of necessary documents and coordinate MDS related IDT meetings in a timely manner.
- Lead and document weekly care plan meetings, incorporating various disciplines.
- Attend scheduled MDS/IDT/Care Plan meetings.
- Ensure daily and weekly documentation requirements on skilled/med part B/LTC residents are obtained and correct.
- Perform other duties as assigned, supporting the facility's mission and addressing overall needs.
- Assist the agency or state government in emergencies as directed by the Governor.
- Work the floor/cart as needed during critical staffing needs.
- Serve in the Nurse on-call rotation.
Requirements
Graduate from an accredited School of Nursing, licensed in the State of Virginia, with two years' experience in long-term care settings or SNFs, and one year as a head/charge nurse or supervisor. Six months experience with resident assessment and MDS forms is also required. CPR certification is mandatory upon hire or willingness to obtain.
Qualifications
Must be a citizen or national of the United States, a lawful permanent resident, or an alien authorized to work. Applicants must pass a criminal background check. Veterans are encouraged to answer veteran status questions and submit their disability documentation.
Skills
Knowledge of RAI process, care planning, and resident assessments, strong communication and documentation skills, ability to manage multiple tasks, and proficiency in computer systems.

Pay
$90,000 - $115,000 annually, with a 12-month probationary period for current state employees restarting upon hire.
Schedule
Monday through Friday, 8:30 AM - 5:00 PM, with a 30-minute unpaid lunch break. Hours may vary based on supervisor's discretion.
