MDS Coordinator LPN

Responsibilities

• Coordinating the Resident Assessment Instrument (RAI) process
• Completing an accurate Minimum Data Set (MDS) and development of the interdisciplinary Plan of Care
• Participating in the daily Medicare Prospective Pay System (PPS) Interdisciplinary Team Meeting (ITM)
• Establishing and maintaining positive relationships with all members of the healthcare team
• Functioning as part of the nursing management team

Requirements

• A current, unencumbered LPN license (as required by state)
• 1+ years of direct hands-on nursing and MDS/RAP experience preferred
• The ability to utilize both local and consultant resources in the execution of job responsibilities
• The ability to develop an adequate knowledge of state, federal, and regulatory requirements related to the job
