MDS Coordinator LPN

Responsibilities

• Responsible for all of the facility Medicare and Managed Care residents as directed by the Resident Assessment Director.
• Oversight of all service provided during inpatient stay from admission to discharge.
• Accurate and timely completion of all OBRA required assessments.
• Viewed as a key financial and clinical member of the management team.
• Not subject to nursing “call” schedules.

Qualifications

• Current, active license to practice as a Licensed Practical Nurse LPN in State of practice.
• Well-versed in inter-personal communications.
• Familiar with the RAI process and Case Management.
• Reads, writes, and speaks English.
• Mets all local health regulation and passes post-employment physical exam if required.
• Meets all local health regulation and passes post-employment physical exam if required.
• Passes drug screening, criminal background investigation, and reference inquiry.

Pay

Minimum pay $44.00. Maximum pay - $49.95. Placement within the wage range for this position is determined by years of experience, qualifications, and demonstrated ability to perform the essential functions.

Schedule

This position is full-time.
