About the role

Hertford Rehabilitation and Healthcare Center is a Long Term Care facility in Hertford, NC, offering quality care to residents. We are seeking a MDS Coordinator to join our growing team.

Responsibilities

• Keep abreast of current federal and state regulations, as well as professional standards.
• Aid in the development of comprehensive care plans for Residents in coordination with the MDS.
• Coordinate, manage, and monitor the written plan of care for each Resident of the facility.
• Assist nursing management with the coordination, management, and review of nurse’s notes.
• Maintain Resident status changes to ensure appropriate and timely nursing or clinical team involvement.
• Ensure MDS and support documentation are accurate and meet regulatory and auditor requirements.
• Perform regular audits of documentation to maintain accuracy.
• Aid in the discharge process.
• Complete necessary administrative forms and reports.
• Maintain infection control and standard precaution practices.
• Follow safety precautions when performing tasks and using equipment and supplies.
• Maintain confidentiality regarding sensitive health information of Residents.
• Report hazardous conditions, damaged equipment, and supply issues.
• Maintain the comfort, privacy, and dignity of Residents.
• Respond promptly and courteously to call lights in Resident care areas.
• Communicate and interact effectively and tactfully with Residents, visitors, families, peers, and supervisors.
• Attend and participate in departmental meetings and in-services as directed.

Requirements

• Current State License as a Nurse
• CPR Certified
• One year experience as a Nurse in a long-term care setting preferred
• Knowledge and training in all aspects of MDS process

Qualifications

Preferred Education and Experience: One year experience as a Nurse in a long-term care setting.
