MDS Coordinator--

General Duties

• Affirms adherence to Federal and State regulations and certifications.
• Participates actively in the regulatory or certification survey process and addresses identified deficiencies.
• Reports trends from completed audits to the Quality Assurance Committee.
• Safeguards the timely and complete execution of the RAI process from the MDS through interdisciplinary care plan completion.
• Maintains tracking of RAI processes including discharge/reentry and Medicaid tracking forms within the PointClickCare system.
• Follows up with staff to ensure compliance with documentation standards.
• Conducts patient assessments, data collection, and interviews as needed to uphold good standard of practice according to the MDS User's Manual.

Qualifications

• Graduate of an approved Registered Nurse program or LPN program and licensed in the state of practice.
• Minimum of 2 years of MDS experience and nursing experience in a Skilled Nursing Facility preferred.
• Thorough understanding of Case-Mix, the Federal Medicare PPS process, and Medicaid reimbursement.
• Knowledge of the Quality Indicator process.
• Understanding of OBRA regulations and Minimum Data Set.
• Knowledge of the care planning process.
