About the role
RegalCare at Glenridge is a skilled nursing and rehabilitation center dedicated to providing high-quality, resident-centered care and achieving strong clinical outcomes. We are currently seeking an experienced MDS Coordinator to join our team.
Responsibilities
- Cook up and execute accurate, timely MDS assessments in line with state and federal regulations
- Manage the RAI process and assist in interdisciplinary care planning
- Ensure that documentation supports quality outcomes and ensures proper reimbursement
- Collaborate with nursing, therapy, and administrative teams
- Assist with audits, surveys, and quality improvement initiatives
Requirements
- MUST have an active RN or LPN license in good standing
- Prior MDS experience in a long-term care setting preferred
Qualifications
- Strong knowledge of Medicare/Medicaid guidelines and RAI requirements
- Excellent organizational skills and attention to detail
- Ability to work independently and manage deadlines
