About the role

Yanceyville Rehabilitation and Healthcare Center is a Long Term Care facility offering quality care to residents. They are currently hiring an MDS Coordinator.

Responsibilities

  • Keep abreast of current federal and state regulations, as well as professional standards.
  • Aid in the development of comprehensive care plans for Residents in coordination with the MDS.
  • Coordinate, manage, and monitor the written plan of care for each Resident of the facility.
  • Assist nursing management with the coordination, management, and review of nurse’s notes.
  • Maintain Resident status changes to ensure appropriate and timely nursing or clinical team involvement.
  • Ensure MDS and support documentation are accurate representations of the Resident and meet regulatory and auditor requirements.
  • Perform regular audits of documentation to assure accuracy.
  • Aid in the discharge process.
  • Complete administrative requirements, such as completing necessary forms and reports.
  • Maintain infection control and standard precaution practices.
  • Follow established safety precautions when performing tasks and using equipment and supplies.
  • Maintain strict confidentiality regarding sensitive health information of Residents.
  • Report all hazardous conditions, damaged equipment, and supply issues to appropriate persons.
  • Maintain the comfort, privacy, and dignity of Residents and interact with them in a manner that displays warmth, respect, and promotes a caring environment.
  • Answer and respond to call lights promptly and courteously when working in Resident care areas.
  • Communicate and interact effectively and tactfully with Residents, visitors, families, peers, and supervisors.
  • Attend and participate in departmental meetings and in-services as directed.

Requirements

  • Current State License as a Nurse
  • C.P.R. Certified
  • One year experience as a Nurse in a long-term care setting preferred
  • Knowledge and training in all aspects of MDS process
