Key Responsibilities
- Cook up and complete MDS assessments (including OBRA, PPS, and any applicable state-specific assessments)
- Ensure accuracy and timeliness of all MDS documentation and submissions
- Collaborate with interdisciplinary team members to develop and update comprehensive care plans
- Monitor case mix index (CMI) and identify opportunities for optimization
- Ensure compliance with CMS guidelines and Minnesota state regulations
- Participate in care plan meetings and communicate effectively with residents and families
- Auxiliary with audits, surveys, and quality improvement initiatives
- Keep abreast of regulatory changes and reimbursement updates
Qualifications
- Current RN license in the state of Minnesota
- Minimum 1–2 years of MDS experience preferred
- Strong knowledge of RAI process, ICD-10 coding, and PDPM
- Experience in a skilled nursing facility (SNF) or long-term care setting
- Strong organizational skills and attention to detail
- Excellent communication and teamwork abilities
- Preferred: MDS Certification (RAC-CT), Experience with electronic health record (EHR) systems
