MDS Coordinator
JOB SUMMARY
Affirms responsibility for the planning, organizing, implementation, and evaluation of Medicare Assessments and Minimum Data Set Plus Coordinator programs. Serves as the Employee Health Nurse for the Brooklyn Gardens Nursing and Rehabilitation Center.
JOB REQUIREMENTS
Education: Bachelor’s Degree preferred.
Certification: PRI Assessor.
Training: Completion of MDS+ Training Course.
Licensure: Licensed in the State of New York as a Registered Professional Nurse.
Experience: At least two years experience as a PRI Assessor in a long-term care facility.
Equipment/Machines/Manual Skills: Required - Computer.
PHYSICAL DEMANDS
Requires standing and sitting for long periods at a time.
SPECIAL SKILLS
Knowledge: Of Medicare regulations in a Residential Health Care Facility.
Skills: Excellent verbal and auditory skills for effective communication with residents, families, physicians, and subordinates. Knowledge of computer.
PERFORMANCE INDICATORS
Timely completion of Medicare determinations.
Informing the Director of Nursing, Business Office, and Social Service of Medicare determinations and discontinuation of coverage.
Completion of Certification and Recertification forms and skilled care forms.
Coordination of the MDS+ form with the Unit Coordinator and related disciplines.
Establishment of facility policies and procedures for key clinical personnel on all shifts.
Coordination of the Employee Health Services in the Nursing Home.
Follows all appropriate safety procedures and respects residents' rights at all times.
Participation in departmental and interdisciplinary committees.
Participation in Interdisciplinary Resident Care Plan meetings.
Quarterly preparation and submission of PRI’s for RUG reimbursements.
Performing all related duties as required and unrelated duties as requested during emergencies.
