Butler Rehab & Healthcare located at 416 S High Street, Butler, MO is seeking an MDS/Care Plan Coordinator to join our team.

Requirements
- High school diploma or equivalent
- 1 year of MDS experience (preferred)
- RN or LPN license
Responsibilities
- Actively participate in the development of a comprehensive, individualized plan of care for each resident
- Ensure resident plan of care is completed, updated, and reviewed in accordance with the RAI guidelines and changes in resident care or needs
- Assume responsibility for implementation of the Clinical and Prospective Payment System Minimum Data Set completion schedule
- Direct, organize, and schedule required resident assessments and related documentation for Medicare, Medicaid, managed care, and Health Maintenance Organization (HMO) reimbursement
- Coordinate and oversee all functions of the MDS assessment process to assure compliance with federal and state requirements
- Participate in resident level of care determination upon admission
- Review reports of timely completion of residents' MDS assessments and care plans and take follow-up action as necessary
- Analyze Quality Indicator reports for potential resident concerns and problems, and review residents' care plans to ensure this information is included
- Maintain a current knowledge base regarding state and federal regulations relating to the RAI process, care planning, and the Prospective Payment System (PPS)
