Responsibilities

• Develop, monitor, audit, and modify each resident’s care plan for their individual needs and goals.
• Perform resident assessments and assist in the discharge process.
• Ensure residents are comfortable and safe by leveraging your knowledge, skills, and attention to detail.

Requirements

• An unencumbered State of Illinois R.N. License.
• C.P.R. Certification.
• As a minimum, one (1) year of experience as an R.N. in a Skilled Nursing Facility setting.
• (1) year of experience as a C.N.A. is preferable but not required.

Qualifications

• Graduate of an accredited nursing program.

• Knowledge of care planning and assessment techniques.
• Attention to detail and strong organizational skills.
• Ability to communicate effectively with residents, families, and interdisciplinary teams.
