About the role

As our MDS Coordinator, you will play a vital role in ensuring accurate assessments, regulatory compliance, and quality resident outcomes. This is an onsite, non-remote position ideal for a detail-oriented nurse who enjoys working closely with an interdisciplinary team.
Responsibilities
- Complete and coordinate MDS assessments in accordance with state and federal regulations.
- Ensure accurate and timely completion of care plans and required documentation.
- Collaborate with nursing, therapy, dietary, social services, and other interdisciplinary team members.
- Monitor clinical documentation to support appropriate reimbursement and regulatory compliance.
- Participate in quality improvement initiatives and survey preparedness.
- Educate and support staff regarding MDS processes and documentation requirements.
Requirements
- Active LPN or RN license required. RN preferred.
- Previous MDS Coordinator experience in a long-term care or skilled nursing setting required.
- RAC-CT certification preferred.
- Working knowledge of Medicare, Medicaid, the RAI process, and PDPM.
- Strong assessment, organizational, and critical thinking skills.
- Experience with PointClickCare (PCC) preferred.
- Excellent communication and teamwork abilities.
