Responsibilities
- Manage the development and implementation of the MDS assessment process
- Ensure accurate documentation and reporting of care plans
- Collaborate with interdisciplinary teams to develop and implement care plans
- Assist in the coordination of care services for residents
- Provide education and training to new staff members
Qualifications & Experience Requirements
- Previous Experience as a MDS Coordinator preferred but not required
- Must possess, as a minimum, a Nursing Degree from an accredited college or university
- Must have, as a minimum, 1 year(s) of experience in a hospital, long-term care facility, or other related health care facility
