MDS Coordinator Responsibilities
- Cook up and implement Individualized Care Plans
- Submit accurate and timely MDS assessments, Care Plans, and other required documentation
- Collaborate with interdisciplinary team members to identify and meet the unique needs of each resident
- Provide education and support to residents, families, and staff on the MDS process and care planning
- Stay current with regulatory requirements and industry best practices to ensure compliance
MDS Coordinator Requirements
- Strong understanding of long-term care services and regulations
- Excellent communication, organizational, and problem-solving skills
- Ability to work effectively in a fast-paced environment and prioritize multiple tasks
- Strong analytical and critical thinking skills
- Able to maintain confidentiality and discretion when handling sensitive information
