Maternal Infant Health Program Nurse Care Manager
About the role
Plans and prioritizes care for individuals and populations of clients, focusing on strategies that will promote optimal health. Provides peer support and role modeling related to the development of life skills and health/wellness to individuals and families during home visits. Demonstrates expertise and current knowledge in patient care and management of a caseload of clients of varying complexity. Seeks to improve patient, family, and health systems/community outcomes through the application of educational concepts/skills and preventive care in a managed care environment.
Responsibilities
- Visit homes to determine client and family needs.
- Perform home health assessments, including blood pressure, pulse, BMI, and foot checks.
- Develop prioritized plans to meet needs and provide services.
- Follow up with families, community agencies, and volunteers to evaluate the effectiveness of services and plan for future needs.
- Assess internal and external referrals to identify patient/significant others’ needs, level of intensity, insurance benefits, and other patient resources.
- Develop plans of care and make recommendations to primary care providers (PCPs), specialists, and other members of the healthcare team regarding care management strategies to maximize continuity of care.
- Assess the educational needs of clients, families, and members of the healthcare team; develop and implement appropriate teaching strategies or make referrals.
- Communicate and collaborate with patients, significant others, providers, and payers to coordinate services that improve access to appropriate services across the continuum of care and promote optimal health cost-effectively.
- Document patient data, plans, interventions, and outcomes according to department guidelines.
- Maintain knowledge of current trends and developments in the field through reading appropriate literature and attending related conferences or seminars.
- Monitor processes and services continuously for quality, cost-effectiveness, and efficiency; engage in process and quality improvement activities.
- Make and implement recommendations to improve operational efficiency and implement new services for areas of responsibility.
Requirements
- Associate's Degree in nursing, social work, occupational or physical therapy, or a health-related field.
- 2 years of relevant experience in the related field.
- State of Michigan Registered Nurse (RN) license (upon hire).
- Basic Life Support (BLS) certification from the American Heart Association (AHA) or American Red Cross (ARC) within 90 days of hire (applicable to Healthier Communities and United Lifestyles only).
- Valid State of Michigan Driver's License (upon hire, applicable to Healthier Communities and United Lifestyles only).
Qualifications
Preferred:
- Bachelor's Degree in a related field.
- Relevant maternal/infant experience.
