About the role
High Point provides a range of community-based services including Children’s Behavioral Health Initiative (CBHI). CBHI services provide individualized, intensive therapy and support to youth and their families. CBHI connects families struggling with mental health and substance use to community resources, enabling them to work towards self-sufficiency. Serving all ages. Home-based therapy provides traditional outpatient individual, couples, and family therapy for adults, adolescents, and children in the home and/or school setting. Our interdisciplinary team is comprised of licensed social workers, mental health clinicians, Master’s level clinicians, and certified addiction specialists. Psychiatric referrals are made when appropriate.
Responsibilities
- Crisis intervention
- Maintain assigned caseload
- Providing direct care to patients/residents – including assessments, individual counseling, discharge and aftercare planning, treatment plans and updates, weekly progress notes, daily groups facilitation, group note completion, and completion of bio-psychosocial assessments
- Making referrals when appropriate
- Responsible for each assigned patients/residents’ medical records
- Connecting patients/residents with aftercare services prior to discharge and aiding the resident and his/her family access services when appropriate
- Communicating/involving family in treatment at patients/residents request
- Attend and participate in all unit, facility meetings, case conferences, Treatment Team meetings, and mandatory trainings, as directed by the Clinical Supervisor /Director
- Responsible for continued education in the field of substance use, mental health and related courses
- Always utilize standards precautions
- Demonstrates behaviors that recognize the rights of patients as defined by the patient’s rights
- Completion of all CBHI New Hire Trainings upon hire (IHT)
- CANS Certified at hire and as recertification is required
- Possesses an understanding of trauma informed care and cultural competence and attends all additional trainings as required/assigned
- Ability to run psycho educational and social support groups as needed
- Knowledge of area resources necessary to meet treatment plan goals of youth
- Ability to network and develop professional working relationships with external area providers
- Place referrals for additional services for youth and families throughout treatment and prior to discharge, as necessary
- Ongoing regular communication and collaboration with other internal and external providers
- Completion of all required documentation at the start of services and updates as required by performance specifications
- Ongoing monitoring and compliance with required documentation for services
- Ability to accurately document time spent with clients, guardians, and collaterals including interventions utilized and responses to these interventions
- Provide clinical services, both in person and through telehealth platform
- Tracking service authorizations dates/units and providing updated information for continued stay reviews, as necessary for the services provided
- Attend all required staff and client meetings, or additional meetings assigned by supervisor
- Attend weekly clinical supervision, tracking all direct client contact and supervision hours toward licensure
- Comply with all necessary medical necessity criteria and performance specifications, dependent on service provided
Requirements
- Must always present as courteous and professional
- Maintains and submits any current Licensure or Certificate necessary for the performance of the position
- Demonstrated flexibility and ability to perform multiple tasks
- Must have excellent communication skills, including the ability to organize ideas in logical and clear fashion
- Knowledge of basic computer skills and software programs including the ability to learn, use and train HPAO Electronic Health Record (EHR)
- Must have the ability to work with others in a cooperative and collaborative manner
- An understanding of the relationship between substance use and mental health issues and/or self- help programs
- Understand and maintain therapeutic boundaries
- Reinforce rules in a respectful manner
- Comply with Federal Regulations, 42 CFR Part 2, Confidentiality of Substance Use Disorder and HIPAA Regulations
- TB/Flu and Covid19 Assessment
- Annual Completion of the HPAO crisis de-escalation program
- CPR Certification
- Successful completion of all trainings upon hire and annually as required
