Manager, Population Health

At Oak Street, Population Health drives both our clinical and economic model and is focused on delivering high-quality, cost-effective care. The Population Health Manager will be instrumental in designing and implementing key Population Health priorities. The Population Health Manager will work on programs and interventions, targeted to the right patients, to improve quality and cost of care. They will be an effective cross team leader, working side by side with Treehouse members and collaborating with providers, care teams, operations team members. Content areas for the Manager will include, but not be limited to, admission prevention, transitions-in-care, pharmacy and medication adherence, HEDIS / Stars, preferred partner relationships and medical cost management. This will require building relationships with our Treehouse team (including Data Science and Finance) and clinicians, including primary care physicians, nurse practitioners, pharmacists, registered nurses, licensed clinical social workers, and others. This is a fast-paced, high visibility role to ensure that we are driving innovation and improvement rapidly and consistently across Oak Street divisions, regions, and centers. Core Responsibilities:Support the execution and the ongoing refinement of Population Health programs.Work closely with functional leaders (Product, Finance, Data Science) in the central office to assist with strategy and program design.Work with regional leaders and Care Teams to ensure effective implementation of key Oak Street clinical and business priorities.Lead communication and content development related to key population health metrics, including quality measures, utilization measures, and financial measures.Other duties, as assigned. Required Qualifications5+ years of applicable experience in consulting / healthcare / population health / working with healthcare providers; or other similar fieldsAbility to build the trust and confidence of a wide range of stakeholdersTrack record of effectively solving difficult problemsPractical, results-oriented mindsetAbility to manage multiple priorities while maintaining a positive attitudeAbility to develop business cases, ROI analysis, and financial analysisSomeone who connects the dots between teams - a proactive communicator who puts individual projects into broader organizational objectivesAbility to succinctly summarize and synthesize large amounts of informationExpert in MS Office, Google Apps, project management softwarePleasantly persistent approachHumilityQuick learner comfortable with a high level of ambiguitySomeone that embodies being “Oaky” Education: Bachelor’s degree required
