Manager, Population Health - Corporate
About the role
At Oak Street Health, Population Health drives both our clinical and economic model and is focused on delivering high-quality, cost-effective care. As a Manager, Population Health, you will play a critical role in supporting our Oak Street Health patients, providers, and business by designing and implementing key Population Health priorities. The Population Health Manager will work on programs and interventions, targeted to the right patients, to improve quality and cost of care. They will be an effective cross-team leader, working side by side with Treehouse members and collaborating with providers, care teams, and operations team members.
Content areas for the Manager will include, but not be limited to, admission prevention, transitions-in-care, pharmacy and medication adherence, HEDIS / Stars, preferred partner relationships, and medical cost management. This will require building relationships with our Treehouse team (including Data Science and Finance) and clinicians, including primary care physicians, nurse practitioners, pharmacists, registered nurses, licensed clinical social workers, and others. This is a fast-paced, high-visibility role to ensure that we are driving innovation and improvement rapidly and consistently across Oak Street divisions, regions, and centers.
Responsibilities
- Support the execution and the ongoing refinement of Population Health programs.
- Work closely with functional leaders (Product, Finance, Data Science) in the central office to assist with strategy and program design.
- Work with regional leaders and Care Teams to ensure effective implementation of key Oak Street clinical and business priorities.
- Lead communication and content development related to key population health metrics, including quality measures, utilization measures, and financial measures.
- Other duties, as assigned.
Requirements
- 5+ years of applicable experience in consulting / healthcare / population health / working with healthcare providers; or other similar fields.
- Ability to build the trust and confidence of a wide range of stakeholders.
- Track record of effectively solving difficult problems.
- Ability to manage multiple priorities while maintaining a positive attitude.
- Ability to develop business cases, ROI analysis, and financial analysis.
- Ability to succinctly summarize and synthesize large amounts of information.
- Expert in MS Office, Google Apps, project management software.
- Pleasantly persistent approach.
- Quick learner comfortable with a high level of ambiguity.
- Bachelor's degree required.

Pay
The typical pay range for this role is $66,330.00 - $145,860.00. This pay range represents the base annual full-time salary for all positions in the job grade within which this position falls. The actual base salary offer will depend on a variety of factors including experience, education, geography, and other relevant factors. This position is also eligible for a CVS Health bonus, commission, or short-term incentive program in addition to the base pay range listed above.
