About the role

Canyon Vista Medical Center is located in the beautiful high desert of southeastern Arizona, serving residents in Sierra Vista, Ft. Huachuca, Cochise County—a population of more than 131,000 people—and surrounding communities. We're comfortably nestled in the San Pedro River Valley, 75 miles southeast of Tucson. Founded in 1963, Canyon Vista Medical Center has grown to a 177,000 square foot facility completed in 2015.

Responsibilities

  • Ensure company-approved programs are implemented and provided consistently.
  • Provide oversight to therapists through performance evaluations, supervision, training, and counseling as needed.
  • Ensure all interventions (individual, group, and family therapy) are performed and documented according to professional clinical standards.
  • Conduct regular audits on patient charts to ensure compliance with Lifepoint and regulatory agency requirements.
  • Actively participate in survey activities (TJC, Medicare, and licensing).
  • Ensure timely completion of all required reporting, including weekly reports, monthly PI and financial audits, engagement and action plans, and quarterly board reports.
  • Attend treatment team meetings regularly.
  • Review with nursing staff to ensure patients are seen by a physician in a timely manner if required by licensure or Conditions of Participation (CoP).
  • Ensure clarification of the therapeutic role and treatment plan with the patient and family is completed and reviewed by each therapist.
  • Actively participate in the complaint resolution process as needed.
  • Oversee therapists to ensure Basis 32 and Patient Satisfaction Surveys or equivalent programs are completed as required by policy.
  • Review transportation issues daily with the Outpatient Coordinator, if applicable.
  • Identify new potential referral sources and maintain good working relationships with all referral sources.
  • Collaborate with other hospital departments and disciplines to ensure patient engagement and connectivity to high-quality care.
  • Partner with the Inpatient Team to support access and engagement in the Continuum of Care.
  • Partner with HR to support and lead retention, recruitment, and succession planning initiatives within the department.
  • Partner with Quality to analyze patient satisfaction data, identify areas for improvement, and lead development opportunities to enhance patient outcomes and satisfaction.
  • Partner with the Business Office, HIM, and UR Departments to identify opportunities for improvement within authorization and denial processes and support or carry out action plans.
  • Obtain pre-authorizations from insurance companies to validate initial outpatient services, if required.
  • Collaborate with leadership team members on hospital operations.
  • Perform other duties and projects as assigned.

  • Attend departmental and staff meetings as required.

Qualifications

  • Master’s Degree from an accredited school of social work or equivalent qualification by education and experience in the social services needs of patients with psychiatric and substance use issues.
  • Current unencumbered Clinical Social Work or Counseling License.
  • CPR Certification and Crisis Prevention Training (CPI) or Handle with Care (HWC) within 30 days of employment and prior to any patient contact.
