About the Role
We are seeking a Manager of Case Management to promote growth and innovation at Sunrise Hospital and Medical Center. This role assists in directing the Case Management program, including case management, utilization management, resource management, discharge planning, and care coordination. You will help coordinate systems for safe and appropriate discharges, ensure utilization management practices meet corporate, division, facility, and regulatory guidelines, and assess staffing levels, competency, and educational needs of the case management team.
This position provides consultation and education to facility leadership, clinical and financial personnel, the Board of Trustees, and medical staff regarding case management practices. You will assist the Director of Case Management in reviewing the annual Case Management/Utilization Management Plan and implementing processes that support efficient patient throughput, optimal clinical outcomes, appropriate resource utilization, patient status, level of care, and reimbursement.

Responsibilities
- Assist in directing the Case Management program, including case management, utilization management, resource management, discharge planning, and care coordination.
- Coordinate systems for safe and appropriate patient discharges while ensuring compliance with corporate, division, facility, and regulatory guidelines.
- Assess and evaluate care/services provided, staffing levels, competency, and educational needs of case management staff.
- Provide consultation and education to facility leadership, clinical and financial personnel, the Board of Trustees, and medical staff on case management practices.
- Assist the Director of Case Management in reviewing the annual Case Management/Utilization Management Plan.
- Identify and implement processes to support efficient patient throughput, optimal clinical outcomes, appropriate resource utilization, patient status, level of care, and reimbursement.
Requirements
- Diploma in Nursing, ADN, BA, or BSN degree in nursing or another healthcare-related field. Master’s degree preferred.
- National Certification in Case Management (ACM or CCM) preferred.
- Minimum of five (5) years of experience in acute care nursing.
- Minimum of two (2) years of management experience in an acute care setting, specifically in case management, throughput, and/or utilization management.
- Advanced knowledge of levels of care criteria, financial management, and regulatory standards for federal payers, JCAHO, and other insurance companies.
