About the role

Supporting the mission of Habitat Health, we are seeking individuals who can help us achieve our goal of empowering older adults to remain in their communities.

Responsibilities

• Engage with residents to understand their needs and preferences.
• Collaborate with healthcare providers to ensure comprehensive care plans.
• Facilitate communication between residents, families, and healthcare teams.
• Ensure compliance with all relevant regulations and standards.

Requirements

• Bachelor's degree in a related field (e.g., gerontology, social work).
• At least 2 years of experience in a healthcare setting or related field.
• Strong interpersonal and communication skills.
• Ability to work independently and as part of a team.

Qualifications

• Valid driver’s license and reliable transportation.
• Proficiency in Microsoft Office Suite.

Skills

• Excellent organizational and time management skills.
• Knowledge of PACE program principles and practices.
• Ability to handle sensitive and confidential information.

Pay

Salary range: $50,000 - $60,000 annually.
