Sunday - Thursday

Responsibilities

• Recruitment, selection, training, supervision, and staffing of team members.
• Implementing resident-centered dementia care.
• Provide nursing care to residents, including but not limited to:
  • Completion of health assessments and resident care plans.
  • Discussing changes in resident condition with physicians.
  • Introducing interventions to reduce falls based on individual resident needs.
  • Medication administration and assistance with resident care, as needed.
• Coordinate the resident admission process including screening, assessment, and care planning in conjunction with the community’s multidisciplinary team.
• Occasionally fill in on the weekend as a Manager-On-Duty.

Requirements

• Current Licensed Practical Nurse license required. Candidates may be considered with caregiver certifications beneficial to the community plus management experience.
• A Bachelor’s degree or higher in Nursing, Social Work, Psychology, or Gerontology preferred.
• Minimum 1-2 years of management experience in directing/providing dementia care for seniors preferred.
• A strong background in patient care assessment, assessing resident behavioral symptoms, and developing non-pharmacologic approaches to meeting resident needs.
• Experience in community or long-term care settings.
• Must meet all state requirements.
