Manager, Advanced Practice Provider - Transitions of Care

About the role

For our client, we are seeking a Manager, Advanced Practice Provider - Transitions of Care to join the team of a leader in the Hospitals & Medical Centers space. In this role, you will lead a team focused on high-quality care during the critical 30-day post-discharge period. You will be responsible for driving clinical performance, improving workflows, and partnering with medical leadership and APP managers to strengthen care delivery and operational outcomes. Location: Remote - US based candidates only, no visa sponsorship available.

Responsibilities

• Provide excellent care to patients in the 30-day post-discharge period
• Oversee and ensure the clinical performance of APPs
• Manage performance of front-line APPs to meet required targets
• Track and manage clinical and operational KPIs
• Identify areas for improvement in team processes
• Conduct bi-weekly one-on-one meetings with direct reports
• Collaborate with medical director and APP managers to enhance workflows

Qualifications

• 5+ years in clinical practice as an Advanced Practice Provider (NP/PA)
• At least 1 year of experience with post-discharge patient care
• 2+ years in management or team leadership roles
• Active RN and APRN license or Physician Assistant license and national certification
• Commitment to a Monday through Friday 9:30am-6:00pm EST schedule

Pay

$121,000 – $155,000 annually
