Managed Care Coordinator (VA Foster Care Program)

About the role

The Managed Care Coordinator role is primarily field-based, requiring travel to client sites or designated locations as needed. Candidates must reside in one of the specified locations: Harrisonburg City, Rockingham Page County, Shenandoah Luray Corridor, Augusta County, Staunton, Waynesboro, Charlottesville, Albemarle, Bedford, Botetourt, Rockbridge, Amherst, Nelson, Alleghany, Covington, Peninsula, Lynchburg, Appomattox, Campbell.

Responsibilities

• Perform telephonic or face-to-face history and program needs assessments using a tool with pre-defined questions.
• Identify members with potential clinical health care needs and coordinate cases with the clinical healthcare management and interdisciplinary team.
• Manage non-clinical needs of members with chronic illnesses, co-morbidities, and/or disabilities to ensure cost-effective and efficient service utilization.
• Establish short and long-term goals in collaboration with the member, caregivers, family, natural supports, and physicians.
• Identify members that would benefit from expanded services.

Requirements

• Bachelor's Degree (BA/BS)
• At least 1 year of experience working directly with people related to the specific program population or other related community-based organizations; or any combination of education and experience which would provide an equivalent background.

Preferred Skills, Capabilities And Experiences

• QMHP, MSW, or BSW degree preferred.
• Experience with Foster Care preferred.
• Behavioral/Mental Health experience preferred.
• LTSS/DD waiver experience preferred.

The salary range for this specific position is $23.75/hr to $37.50/hr, subject to eligibility requirements and geographic location.
