As an Advanced Practice Provider at Theoria Medical, you’ll care for patients over time—building trust, strengthening continuity, and making a lasting difference for vulnerable populations. Responsibilities

• Provide high-quality, patient-centered care to residents in skilled nursing facilities
• Conduct daily rounding, prescribe medications and treatment plans, and address acute or emergent issues
• Communicate with facility nursing staff via phone or secure messaging as needed
• Maintain reliable on-site attendance for scheduled shifts
• Ensure compliance with clinical standards, facility policies, and regulatory requirements

Requirements

• Active U.S. license and board certification by ANCC, AANPCB, PNCB, or AACN (for Nurse Practitioners) or by NCCPA (for Physician Assistants)
• Qualified specialties include Internal Medicine, Family Medicine, Emergency Medicine, General Practice, Physical Medicine and Rehabilitation, and Geriatric Medicine
• Ability to perform essential functions of the position satisfactorily, with or without reasonable accommodation

Schedule

• 6-hour facility shifts for full-time positions (rounds generally start between 7–10 a.m.)
• No on-call or overnight requirements
