LVN - Weekend Shifts

We are seeking a Licensed Practical / Vocational Nurse (LPN/LVN) to provide skilled nursing care in home settings under the supervision of a Registered Nurse. Key Responsibilities Include Delivering and documenting patient care according to physician orders and care plansMonitoring and assessing patient conditions, reporting changes promptlyImplementing nursing actions to achieve desired outcomes and prevent risksEducating patients and families on disease management and self-careMaintaining compliance with policies, regulations, and infection control standardsParticipating in ongoing education and skill development Qualifications Active, unencumbered LPN/LVN license in the practice stateCurrent CPR certificationTB skin test within the last 12 monthsSix months of clinical nursing experience preferredReliable transportation and ability to travel Skills and Abilities: attention to detail, time management, problem-solving, organization, and effective communication. Physical demands include lifting up to 50 pounds, patient repositioning, prolonged standing, and responding to emergencies. Work environments vary and may involve exposure to infectious materials.
