LVN Weekend

Full-time position with a schedule of 6pm–6am.

Qualifications

  • Nursing Degree from an accredited college or university or a graduate of an approved LPN/LVN program.
  • Prior experience in long-term care preferred but not required.
  • Current unrestricted license as a Registered Nurse (RN) or Licensed Practical Nurse (LPN) in practicing state.
  • Current CPR certification.

Responsibilities

  • Directs the daily activities of certified nursing assistants in accordance with current federal, state, and local regulations and facility policies.
  • Ensures compliance with policies and procedures by assigned nursing personnel.
  • Participates in the admission, discharge, and transfer of residents as required.
  • Observes for changes in residents’ status, notifies the physician and resident’s family or representative, and documents accordingly.
  • Transcribes physician orders to medical records and carries out orders as written.
  • Prepares and administers medications per physicians’ orders and observes for adverse effects.
  • Performs wound treatments as per physicians’ orders, observes for changes, and documents accordingly.
  • Schedules follow-up appointments for residents and arranges transportation as needed.
  • Provides nursing leadership to assigned nursing personnel.
  • Performs direct care skills such as colostomy changes, tube feedings, wound care, suctioning, and IV administration in accordance with policies.
  • Performs rounds to ensure resident needs are met and personnel are performing duties.
  • Collaborates with interdisciplinary team members to ensure holistic resident care.
  • Informs appropriate departments of concerns or complaints and files grievances as necessary.
  • Reports incidents or unusual occurrences to supervisors and participates in investigations as needed.
  • Participates in end-of-life care as required.
  • Ensures adequate stock of medications, supplies, and equipment; notifies personnel of needs.
  • Initiates, reviews, and updates care plans as required.
  • Promotes teamwork, mutual respect, and effective communication.
  • Participates in survey processes during inspections.
  • Treats all residents with dignity and respect and promotes their rights.
  • Establishes a culture of compliance by adhering to facility policies and procedures.
  • Complies with safety, hygiene, and infection control policies to protect residents and self.
  • Maintains confidentiality of protected health information.
  • Reports noncompliance, retaliation, discrimination, or allegations of abuse, neglect, or mistreatment.
  • Reports occupational exposures to hazardous materials per facility policy.
  • Participates in life safety and emergency drills and fulfills responsibilities during facility emergencies.
  • Reports work-related injuries and illnesses immediately.
  • Completes all assigned training and skills competency as a condition of employment.

  • Patience and willingness to handle difficult residents, families, and staff.
  • Ability to relay information concerning a resident’s condition.
  • Must not pose a threat to the health and safety of others.
  • Ability to move intermittently throughout the workday and assist in resident evacuation during emergencies.
  • Meets general health requirements, including medical and physical exams and immunity checks.
  • Ability to bend, stoop, kneel, crouch, and perform overhead lifting as needed.
  • May be exposed to falls, burns, odors, dust, tobacco smoke, disinfectants, infectious waste, diseases (e.g., AIDS, Hepatitis B, Tuberculosis), and hostile individuals.
  • Subject to frequent interruptions and may work beyond normal hours, including weekends and holidays.
  • Must follow oral and written instructions and communicate with medical, nursing, and other departments.
  • Works at nursing station and throughout the facility; subject to call-back during emergencies.
